Students seeking credits through University of Primorska, please fill out the following form:

I consent to the University of Primorska (UP) and Oregon State University (OSU) processing my personal and contact information (e.g., email address and mobile/phone number) for the following purposes:

  • administering procedures related to the programmes offered by UP and OSU, including the collection and use of personal documents (such as an ID card, passport, or other identification documents) and photographs;

  • communicating information related to the study process, including programme details, schedules, logistical arrangements, transcript management, and the handling of requests.

I also consent to the University of Primorska (UP) and Oregon State University (OSU) using my personal and contact information for:

  • participation in university promotional activities and the promotion of my curricular and extracurricular achievements online, including on social media platforms.

Taking photographs of me in connection with official and unofficial events related to my summer programme, and using such photographs, with or without my name, for any lawful purpose, including but not limited to publicity, illustration, advertising, and online content.

By providing this consent, I acknowledge that I have been informed of my rights regarding the processing of my personal data, including the following:

  • I have the right to request access to my personal data, as well as its correction, deletion, transfer, or restriction of processing, and I may object to its processing;

  • I may withdraw this consent at any time;

  • I have the right to lodge a complaint with the competent supervisory authority if I believe that the personal data controller has violated data protection regulations;

  • Information about the personal data controller, the data protection officer, and other details related to the processing and protection of personal data is available at www.upr.si;

  • The provided personal data will be retained for a period of five (5) years, in accordance with Erasmus+ regulations;

  • The personal data will be processed solely for the purposes stated above;

  • The personal data will not be disclosed to third parties, except where required by law, contract, or other legal basis;

  • Personal data provided on the basis of consent may also be processed by UP’s contractual partners who process personal data on its behalf and ensure an adequate level of data protection.
